Responsibilities:
	- Prepare for and conduct regular home visits (face to face and/or virtual), averaging 110 units per week.
- Coordinate and conduct the initial assessment using the FROG scale and complete consents.
- Establish collaborative working relationship with the client and family, with the focus on client autonomy.
- Provide parent education using Growing Great Kids provided curriculum. This would include but not be limited to: child development, pregnancy/prenatal education, breast feeding, health, immunization, and well-baby care, language development, home safety, discipline, school readiness, cultural competencies, and family violence.
- Coordinate family support and social services needed by client and family; provide community referrals to other services as needed. Work alongside families to connect to these services in a way that promotes self-sufficiency.
- Complete inventories, forms, Family Goal plans and any other documentation required by the program within the allowable time period as determined by ECS and HMG.
- Complete and maintain accurate written and electronic database records in accordance with ECS, ODH, and HFA.
- Accurately track and document time spent preparing, completing and documenting home visits and utilize good time management skills.
- Identify developmental concerns through screenings and make appropriate Early Intervention referrals.
- Accurately complete required data entry for billing within 48 hours of home visit, per ECS Best Practices.
- Explore future planning with family, including transition to a preschool option prior to child’s third birthday.
- Attend training required by the program, or as recommended by Agency.
- Meet with Clinical Supervisor to review caseload, client progress, forms completion, and training requirements on a weekly basis.
- Attend Team Meetings as scheduled.
- Maintain confidential information regarding clients and Agency.
- Participate in Agency group activities and functions when needed.
- Comply with state and federal regulations that pertain to the position (e.g. OSHA: Occupational Safety Hazard Administration, ORC; Ohio Revised Code).
- Perform other duties as assigned.
Qualifications:
	- Bachelor’s Degree in social work, family studies, early childhood education or equivalent or combination of education and work experience.
- Experience in working with or providing services to children and families.
- Ability to establish trusting relationships.
- Acceptance of individual differences.
- Experience and humility to work with culturally diverse families.
- Knowledge of infant and child development.
- Willing to engage in building reflective capacity (e.g., capacity for introspection, communicating awareness of self in relation to others, recognizing value of supervision, etc.)
- Infant mental health endorsement preferred (if available)
- Ability to work independently and be self motivated.
- Ability to work with various due-dates and support multiple clients at one time.
- Proficient computer skills and basic knowledge of office equipment.
- Detail oriented, strong planning and organizational skills.
- Good time management skills and be able to shift schedules quickly as needed.
- Strong written communication skills and strong interpersonal skills.
- Knowledge of and commitment to a mission-based organization.
- Ability to work as a team member.
- Ability to lift at least 30 lbs.
- Maintain current driver’s license and auto insurance.
- Reliable transportation required.
- Must pass criminal background checks.
Working Conditions
This position requires driving in all weather conditions to complete home visits. There are variances from regular office hours to meet the needs of the clients. Work is performed in a busy environment with numerous interruptions.
Schedule: 40 hours per week including evenings
Pay: $21/hr
